Welcome to a special California edition #ThePossibilists, our roundup of updates from our caffeinated den in Oakland and beyond. This issue look at our recent work with the Camden Coalition related to California Advancing and Innovating Medi-Cal (CalAIM). CalAIM is a five-year, $12B plan to transform California’s Medi-Cal program and to make it integrate more seamlessly with other social services.
Merced CalAIM Conversation May 3, 2023:
Thank you to the Merced and Stanislaus healthcare leaders who joined the Camden Coalition for a dinner conversation about CalAIM. I’m grateful to Golden Valley Health Centers and ClinNEXUS for their sponsorship of the event. Many providers would benefit from GVHC’s lessons learned preparing their FQHC for the CalAIM program. Healthplans and providers agreed that the best approach to CalAIM Enhanced Care Management delivery is a a focus on the fundamentals of care transitions.
