CASE STUDY: COLLECTIVE MEDICAL TECHNOLOGIES
Problem: Form a public-private partnership to introduce a data-sharing platform and collaborative healthcare process to support patients with complex health and social needs.
Solution: Engage leading health researchers and hospital and clinic leaders to define an implementation framework. Develop and execute plan to raise funds, build local stakeholder committee, and launch platform across multiple, unaffiliated hospital emergency department sites.
Success: Launched Northern California’s first-of its kind public-private partnership using a real-time data sharing platform, addressing the unmet needs of over 2,000 Emergency Department users. Generated significant media coverage for participating organizations.
CASE STUDY: CAMDEN COALITION OF HEALTHCARE PROVIDERS
Problem: Stimulate philanthropic partnerships to co-create innovative new models of care and patient programs which focuses not only on those who are ill, but preventive care and saving lives in the local community.
Solution: Develop a seed funding strategy for hospital-based, community-benefit, and philanthropy investments. Create pathway for stakeholders and collaborators to participate in and co-create healthcare innovation at the community level.
Success: Created national partnership between the Camden Coalition of Providers and local East Bay hospitals to serve as national medical advisory site and interdisciplinary hub, training health professional students in the complex care management of undeserved patients.
CASE STUDY: TIPPING POINT COMMUNITY - CENTER FOR YOUTH WELLNESS
Problem: Create joint strategy and business plan among four private and public agencies seeking to align multiple service lines to an integrated health center.
Solution: Provide industry expertise, high-level facilitation, and analysis of critical business. Develop questions for innovative care delivery concept. Conduct iterative individual and group meetings to define core services, options for governance, operations, facilities, and financing
Success: Through research, development, and opening lines of communication, generated support from the founding partners, stakeholders, philanthropists, and policy-makers for the establishment of a new integrated health center.
CASE STUDY: HOPE LAB
FUNDING PLANS
Problem: Need to establish healthcare sector partnerships for product testing and distribution of digital health product.
Solution: Conduct multi-segment healthcare sector landscape research and recommend type and scope of partnerships. Interview contacts with health plans, hospitals, clinics, disease management, and public health to assess market opportunity and key selling propositions.
Success: Developed go-to-market strategy for health care sector channels, including road show and conference schedule to support key meetings. Identified local partnership opportunities with health community-based organizations.
